Works with clients on an individual
basis assessing strengths and
weakness for walking, transferring,

SPEECH
THERAPIST
A Speech Therapist works with individual
clients to provide treatment for speech
disorders. This includes disorders that
prevent people from speaking clearly or
swallowing properly. The therapy helps them
regain their lost abilities. |

OCCUPATIONAL THERAPIST
Works to restore a patient's ability to care
for their own daily needs and regain their
independence. They work on new ways to
accomplish the same old tasks through
adaptive equipment or making the home safe
and usable again for the patient's new
reality. Completely focused on making daily
life work through managing tasks such as
cooking, bathing, personal care and
transportation |

COUNSELING
SERVICE
Our counselors/mental health therapists
provide counseling services that address
depression, anxiety, family issues, grief,
losses, stress, transitions and additional
issues. This treatment is
provided in the clients home or at the out
patient clinic. Insurance/payment is
separate from spell of illness. |

SOCIAL WORKER
Assures the coordination of services are
provided from the community and agency. Also
provides counseling to patients/clients and
family members when needed. |

SKILLED NURSING
A skilled nurse is a licensed registered
nurse or licensed practical nurse educated
to provide general nursing care to
individuals in their own home. Skilled
nursing services that may be provided in the
home include dressing changes, administering
injections or IV medications and
post-hospitalization care. |

HOME HEALTH AIDES
Certified Nurse’s Aides have received
extensive and specialized training suitable
to assist clients who are home bound and
recovering. They augment the nursing team by
providing assistance with bathing. |
