The
Administrator/Owner of First Call
Home Health Agency, opened the
Agency in 2002,
with the vision of
providing high quality healthcare
and a work place that values each
person’s contribution.
Today in 2007 First Call has developed high quality
service to patients that includes a
holistic approach of care providing
great medical care plus teaching and
training to the patient and families
on how to stay healthier longer at
home.
Mullin also served as Vice President
of Community and Senior Services at
Asanti Health System/Rogue Valley
Medical Center, Medford Or. An
integrated health care system
comprised of a 305-bed tertiary,
acute care hospital; physician group
and staff; two neighboring community
hospitals; medical center
foundation, a fund raising division;
home health and hospice agency; home
infusion therapy; outpatient
therapy; 161 bed skilled nursing
facility; Alzheimer care unit;
medical supply company.
Mullin worked on project development
including; reopening a small
hospital through the development of
new geriatric psychiatry program;
chemical dependency facility; adult
medical model day care.
With more than 30 years of management experience in
healthcare Mullin has an emphasis in
home care, including program
development and operations. He has a
Bachelor’s degree in Business
Administration Chadron State
College, Chadron NE. |

Sheryl
Mullin
MA MFT, PC- Owner, First Call
Home Health, Infocus Counseling
Services.
She obtained her Bachelors Degree in Elementary
Education from the University of
Nebraska and her Masters Degree in
Marriage and Family Therapy from
George Fox University.
She has experience in start up business including,
First Call Home Health, Infocus
Counseling and Little House Bakery.
She has obtained state and national
licensure for these businesses and
been involved in most aspects of
negotiating contracts with providers
and developing programs.
She currently serves as President of
the Cambridge Pool Association, and
a committee member for the Growth
Team, Gateway Foursquare Church. |

Jacob Mullin
Director of
Operations
His experience includes working for
Metro One as a telecom engineer,
Network Operations Center Managers
and finished his career there as
Project Manager of a Nationwide
Directory Assistance Network.
Responsibilities included managing a
staff of 15 employees, a
40 million dollar network, 35 call
centers that supported 3500 people,
implementation of the infone network
built to handle 25 million calls per
month.
Jacob graduated from Western Oregon University with a
degree in business administration.
He interned at Salem Chamber of
Commerce.
Volunteer work includes developing and implementing the
Sunday school program for Calvary
Chapel in Beaverton and teaching in
Sunday school class. |

Cynthia Witham
Director of
Community Relations
Her experience includes being the Community
Relations/Marketing Director
for Country Meadows Village,
Keizer Retirement Village
and Mt. Angel Tower. She
started her retirement
industry career with Holiday
Retirement Corporation in
their corporate office.
Responsibilities included
educating the Salem and
surrounding area of the
services each facility
provided, working with
families and individuals to
coordinate move-in dates,
census/occupancy quotas,
public relations,
advertising, maintaining
marketing budgets,
scheduling and implementing
public events and customer
satisfaction.
In the past she has been involved in Salem, Keizer and
Woodburn Chambers of
Commerce. She has also been
Director of Women’s Ministry
at His Way Fellowship and on
the Keizer Chamber of
Commerce Board. |

Lavinia Goto, RN, MBA,
MPH
Director of Professional
Services
Lavinia has worked in health care administrations for
more than 20 years, in all
types of settings, and for
both for-profit and
not-for-profit
organizations. In Oregon,
she spent five years as a
county Director of Public
Health before moving into
her current job as the
clinical director for the
agency. Prior to moving to
Oregon, she lived for 20
years in Hawaii with her
family, working as a
hospital administrator, a
vice president of provider
relations for a third party
administrator, a branch
administrator of government
Medicaid program, and a
director of a community
behavioral health program.
She spent four years
traveling throughout the
Pacific, helping to address
the medical care needs of
people among the
Pacific
Island nations of the
Marshall Islands, American
Samoa, the federated states
of Micronesia, Saipan and
Guam with her home base in
Hawaii. She has also worked
and lived in Japan and
Canada.
She has negotiated contracts with providers of care
from hospitals to
independent Physicians
offices; developed programs
from conception to
implementation: and done
extensive grant writing.
She attended University of British Columbia, University
of Hawaii and University of
Phoenix. She has served on
the Salem Rotary Board and
continues to remain active
with this club as well as
serve on the district youth
exchange committee. She
remains an active member of
the board of Po’ailani, a
behavioral health
residential and outpatient
treatment program for the
dually diagnosed (Mental
Health and Substance Abuse)
in Hawaii. |

Dr.
David Edmunds
Medical
Director
He is currently in private practice as a Family
Practice doctor at The
Doctors Clinic in Salem.
He has lived in Salem since
1960 and started his Family
Practice in 1987. He
is also active staff at
Salem Memorial Hospital
since 1987.
Dr. Edmunds did his undergraduate work at Oregon State
University. In 1984 he
complete his M.D. degree at
Oregon Health and Sciences
University (OHSU).
In 1987 he completed his Family Practice residency at
Phoenix Baptist Hospital,
Phoenix, AZ.
1987 - Board Certified American Academy of Family
Physicians: Recertified
1994, 2001.
Dr. Edmunds volunteer work includes: Adult Committee
member (local board) for
Salem Young Life and on the
Salem Memorial Hospital
Medical Staff Executive
Committee. |
