care program is an essential complement to
our clinical services. In fact, it’s at the
very heart of what we do. Our holistic
care specialists, working as part of the
care team, visit the patient before the
start of care (a non-billed, free service)
to assess the patient’s spiritual, emotional
and social needs. Family and caregiver
involvement is a key to success. Our
holistic care specialists work to keep
families and caregivers informed and
involved in the care process. This service
includes routine follow up to ensure
continued support and progress toward
independence.
