CERTIFIED HOME HEALTH CARE
We are Medicare-certified and state-licensed to offer: Skilled nursing care Physical, occupational and speech therapy Medical social work Home health aide These services are normally paid for by Medicare or private insurance at no cost to the patient.
SKILLED NURSING
A Skilled Nurse is a Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) educated to provide general nursing care to individuals in their own home. A skilled nurse evaluates patients, coordinates care, provides family/caregiver education, disease management, wound care, IV infusion and post hospitalization care.
PHYSICAL THERAPIST
A Physical Therapist (PT) or Physical Therapist Assistant (PTA) works with clients on an individual basis assessing strengths and weakness for walking, transferring, balance and physical endurance. They provide home safety assessments and individualized home exercise programs to decrease the risk of falls.
OCCUPATIONAL THERAPIST
An Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA) recommends adaptive equipment and ways to make a usable safe home environment. They address cognitive and perceptual problems to improve ability to perform tasks of daily living.
SPEECH THERAPIST
A speech therapist works with individual clients to provide treatment for speech and communication disorders. This includes disorders that prevent people from speaking clearly or swallowing properly. They also teach strategies to improve communication.
MEDICAL SOCIAL WORKER
A medical social worker assures the coordination of services provided from the community and agency. Also provides counseling to patients/clients and family members when needed and long-range planning.
HOME HEALTH AIDES
Certified Nurse’s Aides have received extensive and specialized training suitable to assist clients who are home bound and recovering. They augment the nursing and therapy teams.
COUNSELING SERVICE
Our counselors/mental health therapists provide counseling services that address depression, anxiety, family issues, grief, losses, stress, transitions and additional issues. This treatment is provided in the clients home or at the out patient clinic. Insurance/payment is separate from spell of illness.
